Please refer to our Privacy Notice, which details when we can and cannot release your information according to HIPAA guidelines. All patients must sign a HIPAA compliant authorization form. We want you to know that we are committed to doing our best to safeguard the accuracy and security of your health information. We are happy to provide information for insurance or other medically related reasons with proper written authority. We will only release your medical records or information pertaining to your care to you or your designee as documented in your chart.

When requesting copies of your medical records/X-rays, we ask that you please allow a minimum of three business days to prepare your request.
